Functional Assessment Clinic

The Clinic is designed to take a holistic approach at evaluating an individuals needs and current functional abilities, and includes their physical/medical, emotional, mental, and spiritual well-being.

The goal is to either maintain or improve functional skills to maximize independent living capacities and return the individual to balance or harmony. Some of the areas addressed include environmental interventions, nutrition, assistive technology, seating, mobility, transportation, activities of daily living, recreation, arts & crafts, traditional healing, communication, cognitive abilities, medical status and family support.

Many times, a follow up home visit is provided to assess the home environment and living situation. Additional follow-up services are provided to ensure continuity of care, and that recommendations made through the assessment process result in positive outcomes.

The Functional Clinic is provided through the Rehab Technology Team and is a collaborative effort between ASSIST! and Tuba City Indian Medical Center. The Rehab Technology Team includes an occupational therapist, physical therapist, speech/language pathologist, rehabilitation engineer, and a physician if needed.
